On this page you'll find instructions and the health documents you need to fill out as a Grand View student.  See the links at the bottom of the page to download the forms you need.

All athletes, international students and students living in campus housing are required to:

• Print and complete the STUDENT HEALTH FORM.  Turn in the form at the Student Life Office.
• Attach a copy of your CHILDHOOD AND ADULT IMMUNIZATION RECORD to the Student Health Form.
• If you have health insurance, ATTACH A COPY OF THE FRONT AND BACK OF YOUR INSURANCE CARD to your student health form.

Athletes are required to complete and turn in all the forms listed above, and submit them to the Office of the Athletic Director.  You must submit these forms annually.

• Full Acceptance of Risk/Athlete Release 
• Athletic Physical Examination 
• Medical History Questionnaire
• Emergency Contact/Insurance Form
• A copy of your personal health insurance card

Nursing students are required to complete and turn in the forms listed above for all students and also submit the following to the Division of Nursing :

• Check your current Nursing handbook online for immunization requirements for the Division of Nursing. CALL AND MAKE AN APPOINTMENT TO REVIEW YOUR IMMUNIZATION RECORD WITH THE CAMPUS NURSE OR NURSE PRACTITIONER THREE TO FOUR WEEKS PRIOR TO THE START OF CLASSES.
• TB TEST as per the current Division of Nursing Handbook.
• Print and complete the LATEX ALLERGY QUESTIONAIRE.
• HEPATITIS B WAIVER if necessary.
