On this page you'll find instructions and the health documents you need to fill out as a Grand View student. See the links at the bottom of the page to download the forms you need.
All athletes, international students and students living in campus housing are required to:

Print and complete the Student Health Form. Turn in the form at the Student Life Office.

Attach a copy of your childhood and adult immunization record to the Student Health Form.
All international students are required to have health insurance accepted in the United States. You must submit a copy of your personal health insurance card annually.
Nursing students are required to complete and turn in the forms listed above for all students and also submit the following to the Division of Nursing :

If you are without health insurance and need assistance in obtaining it, please contact the Student Life Office at 515-263-2885. Grand View will assist you in locating an appropriate policy. Under the Afforable Care Act, most students under the age of 26 are eligible to remain on a parent's health insurance policy, so you may want to explore that option first if it applies to you.
