Job Description:
Primary/Secondary Customers Primary Customers of the Direct Support Professional are the clients. Secondary Customers are the families of the clients. Principle Accountabilities • Responsible for the health, safety and general welfare of assigned clients for the entirety of each session. • Ability to create and maintain healthy relationships with clients and team members. • Assist with daily personal care needs of each assigned client as needed, including providing support with toileting, teeth brushing, choosing clean clothes, feeding, and all personal care needs. • Acquire working knowledge of each assigned client’s profile, such as individual strengths, barriers, behavior supports needed, medication and side effects. Responsible for documenting the client’s likes and dislikes. • Assist assigned clients with daily morning cleanup, including making beds, sweeping, and sending laundry to housekeeping as needed. • Assist in cleaning of program areas and cabins at the end of each weekend. • Independently transition client groups to scheduled activities, assist clients to be involved in activities, and adapt activities to meet the needs of each individual client providing one-on-one assistance as needed. • Perform duties related to the tracking and upkeep of personal belongings to assure that they are clean, safe and well cared for and available as needed. • Complete all required documentation reviewing client camping application, individual care supports needed and case management plan. • Attend and successfully complete all required training within designated time frame. • Understands, follows and works to ensure compliance with all policies and procedures. • Attend team/program meetings, provide and receive feedback on strengths and weaknesses of planned activities, offer suggestions and participate in general sharing. • Assist with assigned client progress notes. Provide caregiver with accurate information on these forms. • Submit overall program evaluation as requested, citing strengths, weaknesses and recommendations. • Work with other team members in your group to keep them informed of client’s progress and needs throughout the weekend. • Track and maintain position certifications and agency required trainings • Have a working knowledge of Accreditation standards including but not limited to: ACA, CARF, and Medicaid Waiver Services. • Other duties as assigned.
Job Qualifications:
Knowledge, Skills and Experience Requirements • High school diploma or equivalency preferred. • Minimum age of 16. • Desire to work with persons who have disabilities. • Ability to successfully complete orientation. • Ability to pass CPR (Adult and Child), First Aid, Mandatory Reporter within first 90 days • Ability to complete Relational and Technical Mandt training within first 90 days. • Ability to accept supervision and guidance. • Ability to communicate effectively in written and verbal formats. • Ability to write complete sentences and utilize appropriate grammar. • Ability to model Positive Behavioral Supports. • Ability to work with team members and clients in a professional and caring manner, demonstrating integrity and adaptability. • Ability to identify and respond to environmental and other hazards related to an activity; to model and enforces appropriate safety and emergency procedures. • Ability to assess appropriate behavior and apply behavior modifying techniques when needed. • Ability to relate to one’s peer group. • Ability to make decisions using good judgment and common sense. • Ability to bend, stoop, twist and lift • Ability to transfer or move a minimum of 80 pounds. • Must work at least 1 weekend per month, team members are encouraged to work both weekends each month.
