Job Description:
• Oversee, on a daily basis, the personal health, safety and well-being of each client within the guidelines of Easter Seals policies and procedures and Polk County guidelines. • Train and assist clients in the instrumental activities of daily living, including but not limited to cleaning, personal hygiene, leisure and social skills, safety skills, cooking, budgeting, shopping and housekeeping. • Support each client’s somatic care needs including medication administration, when necessary. • Assist clients in maintaining clean, safe and healthy living environments. • Assist clients in searching for independent housing. • Assist clients in obtaining and maintaining educational or employment opportunities. • Train clients in the use of public transportation, including routes, reading schedules, fares and calling for assistance. • Arrange for appropriate transportation, or directly transport clients to and from designated appointments, employment or leisure activities. • Refer and coordinate other funding options for clients, such as Vocational Rehabilitation, Home and Community Based Waiver Services, Medicaid and Medicare. • Remain current on mental health issues and related treatment protocols. Provide adequate supports in the community to divert jail and psychiatric days. • Establish relationships and maintain communication with concerned others, providers and case management agencies. • Monitor each client’s progression toward outcome/goal achievement, including documentation of participant’s activities. • Complete required written reports and paper work • Monitor, evaluate, and track and document overall participant outcomes; make recommendations for improvement. • Carry on-call cell phone for one week approximately every 8th week. • Monitor and work within the constraints of a team budget; make recommendations for improvement. • Learn and implement Positive Behavior Support techniques. • Work flexible schedule/hours including nights and weekends
Job Qualifications:
• Bachelor’s degree preferred or one year experience working with people with disabilities. • Certified in CPR and First Aid within first 90 days • Ability to complete Relational Mandt and Med Management as needed. • Ability to successfully complete Mandatory Reporter of Child and Dependent Adult Abuse within first 30 days • Ability to obtain and maintain Iowa driver’s license (Class D as needed). • Must be insurable under Easter Seals vehicle insurance policy. • Desire to work in a team environment. • Ability to work under pressure, maintain accuracy and meet deadlines. • Ability to communicate effectively in written and verbal formats • Ability to work with wide variety of people, with and without disabilities. • Ability to operate a computer and navigate through Easter Seals database. Experience in word processing software; Word and Excel preferred. • Ability to transfer or move up to 80 pounds. • Ability to bend, stoop, twist and lift • Ability to work non-traditional hours as required, including weekends and evenings. • Access to telephone for on-call system. • Track and maintain position certifications and agency required trainings
