Job Description:
• Monitor, evaluate, and report all needs for assigned clients. Maintain a working knowledge of client’s profile. • Provide skills training for clients. o If in Employment Services the skills training could include but is not limited to interviewing skills, application skills, grooming, community safety skills, social skills and job task development skills. o If in Supported Community Living the skills training could include but is not limited to grooming, community safety skills, and activities of daily living, health care plans, socialization and community involvement. • Schedule community activities/sites for clients as needed per client’s plan. • Monitor and document client’s progression toward outcomes and goal achievement. • Assess clients on going needs and support clients in working towards their goals. • Facilitate and coordinate staffing for clients, preparing staffing reports. • Maintain active communication with all internal team members and with all of the participant’s support team. • Provide referral and establish linkages with various community agencies and services as needed by client. • Provide adaptations to individualized activities and group programming as necessary to teach goal skills. • Complete all documentation as required • Learn and Implement Positive Behavior techniques • Perform other duties as assigned.
Job Qualifications:
• Bachelor’s degree in human services or related field and/or equivalent combination of relevant education and experience preferred. • Experience working with persons with disabilities preferred. • Possession of valid Iowa driver’s license and maintenance of good driving record. • Insurable under Easter Seals vehicle insurance policy. • Access to Reliable Transportation for regular participation in work • Ability to work outside normal business hours, including nights and weekends. • Ability to work cooperatively with team members, clients and the public in a professional and concerned manner. • Ability to work in a team environment. • Ability to communicate effectively in written and verbal formats. • Ability to operate a computer and navigate through Easter Seals database. Experience in word processing software; Word and Excel preferred. • Ability to bend, stoop, twist and lift • Ability to transfer or move a minimum of 80 pounds. • Ability to successfully pass CPR and First Aid within first 90 days. • Ability to successfully pass MANDT training. • Ability to successfully complete Mandatory Reporter of Child and Dependent Adult Abuse within first 30 days
